If you have items you wish to exchange or return for credit, please fill in the form below, print it, and attach it to the clothes you are returning. The clothes you are returning must not have been worn or washed. When you have completed the form, printed it, and attached it to the clothes, please call or email the uniform shop to arrange for an appointment to submit your return in person. If you are unable to take the time to submit your return to the uniform shop in person, please mail your return to Saint Nicholas School Uniform Shop, 10420 Mullins, Houston TX 77096.
